Just like it is described in the book. Adding stuff on top of that is good, but if you are already in doubt about your workings, adding more stuff in just makes trouble shooting the ritual so much harder.
Also, it gives more room for doubt, which is a total killer for any working whatsoever (was it my bornless ritual that botched it all, was my visualization of the middle pillar not optimal? etc.)
Just try it out as it is outlined in the book. Once you get results (no matter how minuscule they are), you can start adding stuff to it to give it more oopmh. And when you feel that one of the additions affects your results adversely, you can easily troubleshoot it and leave it out next time around.
